How to apply: OPTN Committees

Anyone may apply to serve on an OPTN Committee as a volunteer.

To apply:

  1. Log in to the committee volunteer interest form with your username (email) and password.

    First time user? Select Sign Up and follow the prompts to create an account. Be sure to save your username and password.

  2. You must complete all required fields to submit the form. You may return to elaborate on non-required questions at any time.

    You may access and update the form at any time using your login credentials.

If you are interested in serving on an OPTN Committee and need assistance logging in to or submitting your OPTN committee volunteer interest form, please contact unethelpdesk@unos.org

How to apply: OPTN Board of Directors

Read more about the upcoming special election, then complete the following Board interest forms no later than 5 PM ET on April 4, 2025:
